From this page, you report an injury or near-miss that happened to you or someone you know.

Who Can Participate?

Anyone over the age of 18 and involved in agriculture or a related industry can participate.  This includes, ranchers, employees, farm family members, veterinarians, workers in grain handling, ag sales, and custom crop services industries, and others working in ag.

How do I make a report?

To start, click on the “Make a Report” link at the top of the page.  You will see a detailed description of the study that will ask you to confirm that you are qualified for the study (over age 18 and involved in agriculture) and wish to participate. You will then complete a survey about the injury or near-miss you wish to report.  During the pilot study, most people were able to complete the survey in 10-15 minutes. You can skip any questions you don’t want to answer and will be able to exit the survey at any time.

What is being done to protect my privacy?

We ask questions in such a way as to make the information non-identifiable. In other words, it will not be possible to connect you to your report.  We will not open up the database until we have collected 300 reports. All reports will be screened before they are added to the database.  This helps us remove any spam reports and to remove potentially identifiable information if it is accidentally entered in the free response questions of the survey.

Does the injury or near miss have to be recent?

No, we are interested in collecting information about any injury or near miss, even if they occurred months or years ago.

Can I report more than one injury or near miss?

Yes, you can make as many reports as you want.  Please complete a different self-report survey for each injury or near-miss you want to report.

Can I report something that happened to someone else?

Yes, we are collecting both firsthand (something that happened to you) and secondhand (something that happened to another person) reports.
